Our legal duty
Empowermend Wellness & Consulting is required by law to maintain the privacy of your protected health information (PHI), to provide this notice of our legal duties and privacy practices, and to abide by the terms of this notice.
How we use your PHI
We may use and disclose your PHI to provide treatment, coordinate care, bill for services, and operate our practice. We will not use or disclose your PHI for marketing or sell it under any circumstances.
Your rights
You have the right to inspect and copy your record, request corrections, request a list of disclosures, request restrictions on how we use your information, request confidential communication, and file a complaint without fear of retaliation.
Where clinical communication happens
All protected health information is maintained within SimplePractice, a HIPAA-compliant electronic health records and secure messaging system. This website itself does not store PHI.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us at admin@empowermendwell.com or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
